UCHC fails to document all pain medications prescribed

I have reviewed the medical records of a patient that shows the last office visit being 11 months ago, yet the patient has been receiving a regular supply of pain medications MONTHLY since then. Four of the multiple prescriptions were written by Dr. Jay Richards, but NONE of those four are documented in the medical records that were sent to my office. We requested the last two full years of “chart/progress/telephone notes/ALL drug testing AND confirmations/All prescriptions written, samples dispensed, office medication logs, etc./All controlled substance agreements and other associated documentation.” Guess what we got? NO pain agreements. NO Material Risk Notices. No drug tests. They are treating this patient’s knee pain. We requested “All imaging from any sources on file” to help document the patient’s pain issues. Guess what? NO imaging of any type was sent regarding the knee! Wow. I guess the standard of care at these government funded clinics really is lax, even in this time of an “opioid epidemic.” But I guess when you have good insurance, like this patient, you get your pain medications, no matter what. But if you have Oregon Health Plan, you get nothing!
